DDD System Reform:
Changes that Affect your Family
As you may know, DDD is transferring many individuals from their existing funding (including traditional services and self-directed services) to the waiver system: the Supports Program or the Community Care Waiver. Most individuals will be eligible for the Supports Program.
We understand that DDD is in the process of sending letters and inviting you, as family members, to participate in the NJCAT (New Jersey Comprehensive Assessment Tool), also known as the "survey". The DDD letter will provide your individual's MIS number which you will need for future correspondence. The MIS number serves as an identifier for your individual. The letter also provides instructions to complete the survey. You must contact DDD to arrange a phone survey (*see information below) or obtain a password to complete the survey on-line.
Most of you who are in the self-directed funding system are familiar with the survey and know its impact on your family member's budget. *We recommend the on-line option verses the phone survey as this allows you time to process the questions.
After you submit the survey, the result will be that your family member will be assigned a tier, and this tier will align with a budget. You will not automatically receive this information. Once the survey is submitted to DDD you may request the tier assignment by submitting the Request for Tier Assignment form which is located on DDD's website: http://www.state.nj.us/humanservices/ddd/documents/request_for_tier_assignment.pdf
DDD has offered a series of informational webinars for individuals who are in the DDD system regarding the NJCAT and the Supports Waiver. The final DDD webinar of the series will be offered on Saturday, August 15, 9:00 a.m. to 12:00 p.m. Registration is required. See DDD's website for additional information and to register.
